The episode follows a day in the life of Princeton Plainsboro's Dean of Medicine, Dr. Lisa Cuddy, and the inner workings of the hospital are seen through her eyes. This day proves to be especially trying as Cuddy wrestles with myriad hospital issues and staff disputes that test her perseverance and skills as an administrator, all while juggling issues in her personal life. After a tumultuous stab at a mature relationship in Season 7, Cuddy dumps House after a cancer scare which House cannot bring himself to confront without the aid of his old friend Vicodin. Cuddy realizes that House is not and probably never will be ready for a real relationship and ends it.
Characterization
Throughout Season 6, Cuddy is busy with her adopted daughter and is in a relationship with a private investigator, Lucas, who was hired by House to spy on Wilson at the start of Season 5. She cared for House after he goes through rehab for Vicodin. After sensing romantic feelings from House, Cuddy tells House that she would like to be friends; but he refuses, quoting that is the "last thing he wants". In the Season 6 finale "Help Me", House gives Cuddy an antique medical text written by her great-grandfather, which prompts her to confess that she and Lucas were engaged. Cuddy, House and House's team go to Trenton to help victims where a crane collapsed on a building.

Nevertheless, Cuddy manages to stay focused on all the balls she's juggling - putting one aside just long enough to pick up another. Cuddy decided to hire House despite the fact that she acknowledged that he had already been fired from four other hospitals. The exact reason why she decided to hire House has always been kept ambiguous.
Lucas Douglas, who likes Cuddy himself, certainly thinks House is romantically interested in Cuddy, as he saw no other reason for House's attempt to get Cuddy to reveal something about herself to Lucas. House's official explanation was that he was looking for leverage in negotiations, but Lucas rejected that explanation. Their history together is not as clear, but here is what is known for sure. In the episode Moving On, House drove his car into Cuddy's house, permenantly ending their relationship. After this episode, there was no reference to Cuddy having contact with House for the remainder of the series. Following the break-up, House engaged in a series of provocative behaviour designed to make Cuddy jealous, including marrying Dominika Petrova in the hospital. In the episode Known Unknowns, while attending a medical conference, House admits to Cuddy that he had feelings for her when he first saw her at the Michigan University library, catching her off guard. The next day, he went up to her hotel room to ask if he could help with her baby, only to find out that Lucas was in the room with the baby. Cuddy and Lucas tell House they started going out right after House was admitted to Mayfield Psychiatric Hospital. Later, in the episode Ignorance is Bliss, House tries to break up Cuddy and Lucas. Cuddy invites House to her home for Thanksgiving dinner, but gave him the wrong address to stop him from ruining the evening.
